First time posting (systems consultant) - Dev's on holiday so seeing if I can fix this :-) I cleared my cache today and key entity lookups are showing an icon but not the record's name. I've checked the primary columns on the entity lookup views and they are set as Name. No customisations have been published by us since this change.

I have the same issue across multiple sites and have raised a ticket with Microsoft. Appears to be the result of an update pushed over the weekend.
